PHSC is a Patient Centered Medical Home (PCMH) and is accredited by the National Committee for Quality Assurance (NCQA). We provide a team-based approach to providing health care services that create a partnership between the patient, health care providers, and health care team. Patient Centered means that you are the focus of your health care. Together, following evidence-based guidelines for medical and behavioral health care, our team will provide the best health care treatment for you. PHSC does not exclude people or treat them differently because of race, color, national origin, age, disability, or sex.
No one will be denied access to services due to inability to pay and there is a discounted/sliding fee schedule available based on family size and income.
