Primary Health Services Center (PHSC) is a private, nonprofit health care organization founded in Monroe, Louisiana in 1997. As a Federally Qualified Health Center, we provide comprehensive, community-based care designed to serve individuals and families across North Louisiana—especially those in medically underserved areas.
Our approach goes beyond treating symptoms. Through a patient-centered model of care, we combine medical, behavioral, dental, and supportive services to improve overall health and remove barriers to access—ensuring every patient receives the care they deserve.
